MSYSA Registration Procedures

1.      All adults officiating on a team must independently register before the team can be created. https://www.youthleaguesusa.com/msysa/10-11/AdultVolunteer.html

2.      The Team Manager creates the team at https://www.youthleaguesusa.com/msysa/10-11/TravelTeam.html  If the Adults have not registered as in step 1 above the Team Manager will not be able to add them to the team roster.

3.      Once the Team is registered the Team Manager will be emailed a unique user ID and password.

4.      Login at https://www.youthleaguesusa.com/msysa/10-11/Database.htm

5.      From the top left hand side select “Officials” from the data type. You should be able to see all officials associated with your team. If someone isn’t listed who should be it means they have not completed the Adult Registration.

6.      Select “Team” from the data type at top left.

7.      Select the button “Enter last part of team ID” and enter your team number given to you by the league with which you are registered.

8.      Select “players” from the data type at top left.

9.      Select button “Add new primary player” and enter all required data that you have gathered using the Player Registration Form http://www.msysa.org/docs/registration/forms/mem11reg.pdf  Repeat this process for each player.

10.   After all players have been added, jersey numbers are all entered, you are ready to print your roster. After selecting “print roster” you’ll be shown the correct margins etc to set.

11.   Next you need to print ID cards. Select the correct card stock you are using-either “blank member passes” or “member passes with heading”. Both are acceptable as long as all of your passes are the same format.

12.   Team Managers do not need to do anything with regards to approving proof of age – this is done by the Registrar.

 

To Multi Roster a Player

 

1.      Have the player and players parents complete the Player Registration Form http://www.msysa.org/docs/registration/forms/mem11reg.pdf

2.      Once logged into the team database select the “Add New Multi Rostered Player” button. You will be asked to enter the players full first and last name and their date of birth. The system should located that players registration record for you.

3.      Multi  Rostered players have yellow ID cards that are available from your Registrar. Have the player sign their full legal name on both cards and attach photos.

4.      The Registrar will hand write the MR card.

 

What to take to the Registrar:

 

For each primary rostered player:

-        Original registration form plus 1 copy (hand written or printed from the database are acceptable)

-        2 part player pass signed and with photos attached - correct photo and signature placement is shown here http://www.msysa.org/docs/playerregistrationforms/packgprep09.pdf  Also note that player passes must show the legal name of the player. Robert Brown must be printed and signed as Robert Brown – not Rob or Bob. It must match what is on the player’s birth certificate or other proof of age document.

-        Proof of age:

Returning players – previous years ID passes

New players – original birth certificate (copies NOT accepted)

 

 

For each rostered Adult Official:

 

-        Original registration forms plus 1 copy (must be printed from the database)

-        2 part adult ID cards signed and with photos attached - correct photo and signature placement is shown here http://www.msysa.org/docs/playerregistrationforms/packgprep09.pdf

 

Multi Rostered Players:

 

-        Original registration forms plus 1 copy

-        2 part yellow MR card signed and photos attached

-        Copy of Registrar approved and stamped primary player card or registration form.

 

 

Stacking of your team packet: 

 

Place original roster followed by 2 copies of roster

Player registration forms (in order as listed on roster)
Paperclip copy of registration, new pass/old pass or birth certificate to original player registration form
Place all items in an envelope labeled with Club name/Team name/Age/ Team number/Contact info.

 

 

Once your packet is returned to you the Team Manager must arrange to have all ID cards laminated before game play. This is to be done at the teams expense.
